Please open up the Application Form by clicking on the above image. Please fill out and complete the entire Application Form including three (3) personal references and the Release of Information Sheet. Once you complete the application please email it as an attachment to the following email addresses:

Upon receiving your application, Mr. Ed Wacker will create your file and then call you to set up an interview at our office which is located at:

Interviews will begin in late August 2013 through the first week of October 2013. Our four (4) Saturday Fall 2013 Pre-Service Training Classes for new guardians are scheduled for October 5, 12, 19 and 26, 2013.
